STEP 1

Prepare the following six items. Follow the instructions for each item:

♦ IP Waiver Forms
You are required to submit a signed IP Waiver form and to have the IP officer of your organization sign a similar form. Note: The two forms are not identical – be sure to sign the correct form and to share the correct link with the IP officer.
Instructions:
Once the forms are completed and signed, please upload them where indicated in the application form below (SECTION 6).
1. Applicant Waiver: Click on this link and complete the required fields.
2. Organization (IP Officer) Waiver: Send this link https://isoh-tech.org/waiver-ip-2025-organization/ to the IP officer of your organization, asking them to send the signed document back to you.
♦ References
You are required to provide 3 references as part of your fellowship application.
One must be from the director of your department or organization, and one should preferably be from a colleague.
Instructions: Copy this link https://isoh-tech.org/fellowship-recommendations-2025/ and send it to each of your references.
Please ask each reference to submit the form within 1 week of submitting your application. Please follow up with them to ensure that the references have been submitted on time.
♦ Fellowship Program Schedule
Instructions:
1. Click here to review the schedule to ensure that you will be able to commit to participating in at least 90% of all activities throughout the year.
2. Click on this link to download the commitment form, complete the required fields and sign.
3. Once the form is completed and signed, please upload it where indicated in the application form below (SECTION 6).
STEP 2
Understand how the application form works.
♦ Saving your application form.
You might not be able to complete the form in one session, so you can always save your incomplete form and go back to it later.
Instructions:
1. At any time just click on “Save and Continue Later” at the bottom of the form.
2. After clicking on “Save and Continue Later”, be sure to give your email address to receive a unique link that you will use to go back to your form when you’re ready.
3. Important note: You will be asked to upload documents towards the end of the form. Upload your documents only when you are ready to submit the complete form as they will not be saved when exiting an incomplete form.
♦ Many fields are required.
If there are any required fields (marked with * ) that have not been completed when clicking “SUBMIT”, you will see notifications on the form page indicating the fields that need to be completed. You will need to scroll down to the form to see the fields to complete.
♦ Confirmation when completing the application:
When you have successfully completed and submitted the application form, you will immediately receive confirmation and our team will contact you soon after.
