Instructions: Please send this form to 3 different references. One of the references must be the director of your department or organization.
Please ask each reference to complete the form and return it directly to us at fellowshipht@gmail.com by June 29th, 2023.

Instructions: Review the schedule to ensure that you will be able to commit to participating in at least 90% of all activities throughout the year. Your agreement to this is required in the application form below.
STEP 2
Understand how the application form works.
♦ Saving your application form.
You might not be able to complete the form in one session, so you can always save your incomplete form and go back to it later.
Instructions:
1. At any time just click on “Save and Continue Later” at the bottom of the form.
2. After clicking on “Save and Continue Later”, be sure to give your email address to receive a unique link that you will use to back to your form when you’re ready.
3. Important note: You will be asked to upload documents towards the end of the form. Upload your documents only when you are ready to submit the complete form as they will not be saved when exiting an incomplete form.
♦ Many fields are required.
If there are any required fields (marked with * ) that have not been completed when clicking “SUBMIT”, you will see notifications on the form page indicating the fields that need to be completed. You will need to scroll down to the form to see the fields to complete.
♦ Confirmation when completing the application:
When you have successfully completed and submitted the application form, you will immediately see a confirmation page: “Thank you for submitting your Fellowship application” and our team will contact you soon after.
